Fun fact: According to Dark Souls lore, Zweihänder means "two hander" in an ancient language.

We don't know how many kingdoms got merged into one, or where they were. What we know is that over time in the Souls universe, they move closer to one another, sometimes even merging or being misshapen. So yeah, maybe Germany existed and got absorbed by Lothric or something.
