As a primary factor, running a clinic with a population of 806 with the current payout structure of Medicare, Medicaid, and insurance companies is frankly unsustainable. It's not wrong to say the Medicaid cuts are not the primary factor, just the last straw in an already broken model. Giving service to a community that small and spread out takes state subsidies or the regional health care system eating the losses. It sounds likely this crew would be hostile to the former and are mad that the nonprofit can't continue to afford the latter as the Medicaid cuts squeeze them more.
Much of these services are offered primarily to avoid 30 day readmission penalties that CMS hits hospitals with after discharging people to areas with no follow up care. It essentially externalizes the cost of providing this care into regional health systems. But eventually they'll find other avenues, like very targeted home visits and increasing nursing home admissions, as the costs get unsustainable. And then the costs of this lack of access are externalized right back onto the community.
It's not even don't get in. They're rigorous institutions that have a marginal difference in competitiveness. They just make you take a class on those dumb manipulations. In the end it doesn't matter if you went through DO school or med school: if you're shit you're not getting matched into or getting through residency. And the key part is both sides of that pipeline compete for the same limited residency placements that are significantly fewer than the number of graduates and are judged the same.
I do a similar trick as a shirtless fat guy wearing flip flops cycling in the middle of the road in my city. In my case it's less that they're not so aggressive but more that they end up being convinced that I'm a crazy person they should give space to. Which I am.

I also don't think that geothermal power installations stand to solidify the earths core and remove our magnetosphere. I just think it's cool that it's literally where the power is coming from.
Yeah and any friction from tides(instead of free movement) slows the rotation and pushes the moon away. It's using the leverage of the gravity to perform work against the planet itself. Devices like underwater turbines, by extracting energy from the tidal currents, effectively increase this friction, which in turn amplifies the effect. The amount of energy naturally pulled out of this system is entirely dependent on the size of the tidal bulge and the shape of the seafloor/coast.
As for geothermal, we do use it, but only in hot spots.
